Patient Information

Steps to Surgery

Step 1
Attend a seminar at MTMC's Bariatric Center (click here for dates)

Step 2
Complete the "Patient Questionnaire" and return to The Bariatric Center via in person, by mail, email, or fax (615.890.2820).

Step 3
Go see your medical doctor for the following:

  • Physical exam and clearance for bariatric surgery
  • 1 office note per year with weight x 5 years (2006-Current)
    A MUST BEFORE STEP 4!
  • 10% or 6 month weight loss program with PCP or Dietitian
  • Letter of Medical Necessity
  • Lab work: CBC, CMP, Iron, Ferritin, Vitamin D, PTH
  • Cardiac/Pulmonary clearance letters from specialist if this applies to you.

Step 4
Schedule a Psychological and Nutritional evaluation:

  • Elysse Beasley Senior Psychological Examiner 615.896.9160
  • Heather Wanke Nutritionist 615.867.1940 Ins. Not accepted, $75.00 fee (Must Be Seen)

Step 5
Attend (1) support group at MTMC's Bariatric Center. (Click here for dates)

Step 6
Contact the Bariatric Insurance Specialist (Shavon 615.396.4965) to ensure all the necessary paperwork has been received. Once received Susan (615.867.1940) will contact you for your consult with Dr. Eckles or Dr. Westmoreland of Murfreesboro Surgical Specialist.

Final Step
Once approved, Shavon or Anne Marie will schedule a Pre-Operative class at MTMC's Bariatric Center. Stacy at Murfreesboro Surgical Specialist will contact you for your weigh in appointment with the surgeon.

Aftercare Programs

Bariatric surgery is only one small component of the overall success for weight management.  Physical activity, good nutritional habits, and smoking cessation are essential to a patient's long-term weight maintenance and health.  The most successful long-term surgical weight loss patients are committed to exercise.  We want to help you achieve and sustain your personal goals while providing encouragement, support, and direction every step of the way.

Participants meet one-one-with an Exercise Physiologist to:

  • Discuss and record personal goals.
  • Develop a plan to achieve your goals.
  • Develop a cardiovascular and strength training program.
  • Evaluate your program on a regular basis to adjust, as needed.

For free exercise consultations for life, participants do not need to be a member of the Wellness Center to meet one-on-one with an Exercise Physiologist.  A general or detailed program information. Prior to participation, a physician's referral and release is required. To participate in this program, call the Wellness Center at 896-0290 or stop by the front desk.
